My mountain bike front derailleur is rubbing in every gear. It gets into each gear just fine, but when the chain isn't running nearly parallel with the gears, it rubs. I tried turning the barrel adjuster about two complete rotations in each direction, but neither helped.
Is this a cable related problem or is there a chance my front derailer itself has rotated and no cable adjustments would help?

Alan,
Which side of the cage is the chain rubbing on? And which chainring?

Standing atop the bike, it rubs on the left part of the cage. In all front gears. Except when I shift in the back to get the chain in a more parallel configuration. Any time there's more than a gear or two off center, it starts to rub.

Sounds like the cable tension is to high or the derailleur has been rotated.
